Review the letter and follow the instructions. You may need to renew your coverage or provide additional information.

I do not understand the letter. Can I get help?

Yes. A B – UFC Community Assistor can review the letter with you and explain what is needed.

What happens if I do not renew by the due date?

You may lose your health coverage. It is important to respond by the deadline provided.

Can I get help completing my application or renewal?

Yes. Assistance is available to help you complete forms and understand required documentation.

What if my address, income or household has changed?

Changes must be reported as soon as possible. Assistance is available to help you update your information.

What documents may be required?

You may be asked to provide documents such as proof of income, identification or residency.

I do not use a computer. Can I still receive assistance?

Yes. Assistance is available by phone.

Can a family member or caregiver assist me?

Yes. A family member or caregiver may assist you. We may also speak with them with your permission.

Is there a cost for Community Assistor services?

No. Services are provided at no cost.
