Pharmacy Prior Authorization

The Plan requires you or your physician to get prior authorization for certain drugs. This means that you will need to get approval before you fill your prescriptions. If you don’t get approval, we may not cover the drug.

How to Request Prior Authorization:

  1. Download and complete the form – You or your provider can download the Prior Authorization Request Form to submit the necessary information

  2. Submit the form – Follow the instructions on the form for submission
  3. Wait for approval – Most requests are reviewed within 72 hours for standard requests, or 24 hours for urgent requests
