If you have had blood clots or are at risk for blood clots, the team at the Anticoagulation Clinic will help you maintain your diet and manage your medications.
When you visit the clinic, you’ll first have blood work done. You’ll also have a one-on-one with a pharmacist where you’ll talk about any recent illnesses or lifestyle and dietary changes. With this information and your blood work, the pharmacist will determine if any adjustments are needed. After your appointment, the pharmacist will update your primary care doctor to keep him or her in the loop.
