Below are answers to frequently asked questions about our acquisition on May 1, 2025, of several Village Medical primary care and walk-in clinics across Fort Collins, Longmont, Loveland and Windsor - a step that strengthens our commitment to making health care easier and better for our northern Colorado communities.
How do I access my medical records?
For medical records for services prior to August 1, 2025, you may use this link to request your medical record from Village MD.
You can view a summary of your health information through your Banner Health (portal) account. You may also request a copy of your medical record through the portal or through our electronic request.
How do I refill a prescription?
For prescription refills, please contact your pharmacy. They will coordinate with your Banner clinic to process your request.
What insurances are accepted by Banner Health?
We accept most major insurance plans. For peace of mind, we recommend contacting your insurance provider to confirm coverage details.
What does this mean for my upcoming appointment?
All existing appointments will be moved into the Banner Health system. If there are any changes to an upcoming appointment, you can expect a call from your clinic. Beginning Aug. 1, 2025, you can schedule appointments by calling your clinic directly and speaking with our local schedulers. Online scheduling will be available at a later date.
What appointment notifications can I expect?
Starting Aug. 1, you'll receive appointment reminders 7 days, 2 days and 30 minutes before your scheduled visit, with options to complete pre-appointment information. Depending on your appointment date, you may not initially receive all notifications.
Where will lab services be provided?
Laboratory services will continue to be conveniently available at all clinic locations. We are transitioning from LabCorp to Horizon Laboratory as our primary lab service provider. We recommend verifying coverage for laboratory services with your insurance provider to ensure a smooth experience.
