Banner Health is taking a leadership role in two new patient safety initiatives designed to save lives and improve health care. The two initiatives, The 100,000 Lives Campaign, and The Med Form, make patient safety a top priority.
The 100,000 Lives Campaign
This voluntary initiative is designed to make health care safer by ensuring the best possible outcomes for all patients.
Under the campaign, hospitals and health care organizations pledge to implement specific, evidence-based quality improvement changes and report on implementation progress. As a system node, Banner has committed to serve as a resource for other health care organizations participating in the campaign.
The 100,000 Lives Campaign involves health care providers in implementing six strategies to avoid preventable deaths. Banner Health’s role as a node in the 100,000 Lives Campaign, positions the company as a proactive leader playing a central role in driving the improvement work of each campaign network.

Banner Health’s newest patient safety initiative is a partnership with the Arizona health care community to introduce a consumer tool called The Med Form to reduce medication errors. Available in English and Spanish, The Med Form is a simple, free document that provides consumers with a single record of all medications – prescriptions, herbals, over-the-counter, and vitamins, as well as immunization and allergy information.
The Med Form is tool for medication reconciliation, which is a process of identifying the most accurate list of all medications a patient is taking and using the list to provide correct medications for patients anywhere in the health care system – doctor’s offices, hospitals, pharmacies and elsewhere. The Med Form is designed to reduce medication errors by promoting communication between consumers and their care givers. Consumers will keep an up-to-date copy ofThe Med Form and present it to their doctors and hospitals at each health care visit to provide an accurate medication history for inclusion in their medical records.
Up-to-date information is critical whether in a medical emergency or a planned physician visit. Patients are the most important component of the medication reconciliation process because they are the common denominator as they move through the health care system, whether for pre-hospital professionals at the scene of an accident or physicians in their offices.
