As directed by the federal Medicare program, Banner Medicare Advantage plans are available to individuals who reside in the following counties (called “service area” by Medicare):
To remain eligible for Banner Medicare Advantage membership, you must continue to reside in the service area. If you plan to move out of the service area, please contact our Customer Care Center. When or if you move out of the service area, you will have a “Special Enrollment Period,” enabling you to review and select a new Medicare health and/or drug plan available in your new service area; the Original Medicare plan is always an option as well.
Also, remember to call Social Security if you move or change your mailing address.
