Request a Transcript of CME Credit

For transcript requests, please contact the Banner CME office at (480) 684-5560 or by e-mail with the following information:

  • First and last name, including given name as well as name commonly used
  • Date range for transcript request (for example, credits from January 1, 2018, through today)
  • Specify email address to which report should be sent
  • Provide a phone number or email address for clarification purposes

Please allow five business days to receive your transcript of CME activities.

Privacy Information

The Banner Health CME office respects your privacy and is committed to the protection of personal information you share when participating in CME activities.

As part of the CME activity, you may be required to complete a test and/or evaluation questionnaire. Your individual responses will not be shared; however, they can be accessed by the activity director and Banner CME staff. Evaluation data is used to assess the educational value of the activity and make improvements to the overall CME program. Aggregate reports based on evaluation data will not include identifiable personal information.
