- Please enter the facility through the East Lobby Entrance, located on 12th Street just south of McDowell Road. For your convenience, there is valet parking or a patient parking garage available.
- Please bring a family member or caregiver with you to your appointment.
- Please bring all of your current medications with you to your visit.

Payments Accepted
Patients are asked to pay their insurance co-payment at the time of their visit. We accept Visa, MasterCard, American Express and Discover as well as personal checks or cash. Any outstanding balance is due within 30 days.
