For patients with chronic conditions, such as congestive heart failure and diabetes, we offer our Virtual Home Monitoring program. Our highly skilled care team works with patients to help them monitor their health and live life to the fullest.

What Is Home Monitoring?

Virtual Home Monitoring is a service for patients who need assistance monitoring their chronic conditions daily. This program allows patients to learn about their medications and how to manage their condition at home, while still receiving the support they need from our home care nurses.

Through our Virtual Home Monitoring program, patients and caregivers learn to take and track their vital signs every day. The vitals are stored in a monitoring device that sends the information to a secure website.

Through this secure system, nurses are able to monitor and assess the data and contact the patient if they notice anything unusual. If necessary, the patient’s physician will be contacted, an in-person home visit may be done or the patient may be told to go to the emergency room if the symptoms are severe.

Patient home monitoring is recommended for patients with chronic diseases or injuries that require long-term care such as:
