With one call, Banner Health Transfer Services assists with the placement of patients who are experiencing illnesses and injuries across the health continuum, often reducing admissions through already overcrowded emergency departments. Whether your patient needs a higher level of care, are facing one of the most complex medical conditions, or are in need of specialty services not otherwise available, Banner Health Transfer Services can locate the appropriate physician and facility for any adult or pediatric patient. After placing your phone call, our experienced triage team consisting of both registered nurses and administrative staff will coordinate the complete transfer process including:

  • Connecting/consulting with a Banner Health physician
  • Arranging for a patient bed
  • All other patient transfer needs

If while consulting with our provider you jointly decide an admission is needed, a nurse will take clinical information to safely place your patient at the right level of care, at the right facility, often with less financial liability for the patient.  We are here for you 24/7, and look forward to serving your patients and our community.

Banner Health Transfer Services will request the following information for consultations, admissions and transfers:

  1. Patient’s first and last name, date of birth, gender, and diagnosis
  2. Name of your referring physician and their call back phone number
  3. Which service is your physician requesting to speak with?
  4. Based on the patient’s condition, is this considered routine, urgent, or emergent?

The additional information will be needed for admissions and transfers:

  1. Phone number to call with the bed assignment
  2. A Banner Health Transfer Services RN will consult with your provider to discuss clinical information relevant to transfer and continuity of care
