Our Banner Family Pharmacy team makes it simple to move your prescriptions. You can transfer medications online or by phone, and we’ll take care of the rest. We’re here to support your health and ensure you get the medications you need without delay. 

How to transfer a prescription 

Choose the option that works best for you: 

Transfer by phone 

Follow these quick steps: 

1. Find the prescription number or medication name on your current bottle. 
2. Locate the phone number of the pharmacy where your prescription was last filled. 
3. Call the Banner Family Pharmacy location you want to use and share that information. 

Our pharmacy team will contact your previous pharmacy and handle the transfer for you. 

Transfer online 

You can also request a prescription transfer using our secure online form. 

1. Select your preferred Banner Family Pharmacy pickup location. 
2. Enter your prescription information, including the current filling pharmacy. 
3. Provide basic personal details like your name, date of birth and phone number. 

Once we receive your request, our team will complete the transfer and notify you when your medication is ready.
